so tony’s never known anyone who he actually believes likes him. everyone in his life is paid to be there. they’re there to get something out of him, money, fame, tech.
and he’s grown up with distant and cold parents, and probably a series of nannies who were uncomfortable and detached because tony wouldn’t have been a normal kid, he would have been unusually smart, and more interested in building things and engineering than normal, and so these nannies - and it’s not really their fault - wouldn’t have connected with him as much they would have a regular boy whose favourite things were his teddy and racing cars, not his wires and circuits. so tony would hardly have been hugged, and if he was it wouldn’t have been warmly.
the only real contact he would have had with his parents was when they were in public, presenting the face of a loving family. and his father beforehand would have taken him aside and told him not to make a fuss or complain or behave like.. himself and just to smile and give the people what they want. and so tony did, and howard would ruffle his hair and laugh and smile at him.
and we know that tony stark billionaire genius playboy philanthropist is a mask

and it’s one that tony puts up still, even though howard isn’t alive to tell him to set aside himself and don a smile
he does it because some part of him believes that it’s the only way people will ever like him. his parents only seemed to care about him when he had it on. whether consciously or not, tony believes that the mask is the only thing that’s going to bring him any level of affection from anyone else, because it was the only thing that did when he was a kid
because the world loves tony stark, but nobody loves tony
